School Physical Examinations
The Williams Bay School District requests a physical and a dental check for all 4k (kindergarten) third, sixth, and ninth grade students, in addition to new district students. Physical forms are to be turned in to the school office by the first day of school. A student physical form can be picked up in the school office or downloaded from our website.
School Physical Examinations for Sports: The sixth grade physical is required for middle school student participation in school-based after-school athletic programs (football, cross country, basketball, etc.). WIAA rules require high school athletes to have a physical every other year. The ninth grade school physical is accepted as a sports physical for 11th grade for sports.
School Dental Examinations
The Williams Bay School District acknowledges the importance of regular dental exams and the effects of dental health on the overall health and success in schools. Parents are to provide a minimum of four dental exams during their child's twelve years of schooling. Specifically, exams are requested for 4K/kindergarten, third, sixth, and ninth grade children and for all children who transfer into the Williams Bay schools. Dental exam forms are to be turned in to the school office by the first day of school. A dental exam form can be picked up in the school office or downloaded from our website.

Wisconsin State Law (s.188.135) Concerning Eye Exams

The Williams Bay School District is required to request each student entering kindergarten to provide evidence of an eye examination by a physician (ophthalmologist, optometrist, or general physician). The State of Wisconsin Eye Exam form is provided at kindergarten registration and is also available online or in each school office. This form documents that a complete eye examination was performed, not just a vision screening. Specifically, the State of Wisconsin encourages parents of kindergartners to arrange for their child's eyes to be examined by an optometrist or evaluated by a physician by December 31 of the child's first year in school. An examination or evaluation should include, at a minimum, the elements listed below.

Immunizations are considered one of the greatest medical achievements for the 20th century. Vaccines prevent disease in people who receive them and protect those who come in contact with unvaccinated individuals. Diseases that once killed many children and adults are rarely seen today because of immunization. But these diseases are still around, especially in other parts of the world. Vaccination continues to be one of the most important things you can do to protect your child and yourself from dangerous disease The Williams Bay School District is requesting that parents follow the immunization guidelines set forth by the CDC and the State of Wisconsin.
